Medical Release Forms
On this page, you’ll find all required medical release forms needed for participation in competition.

The Athlete Physical Examination Form is required annually to confirm an athlete’s medical eligibility for participation. Once completed, the form must be uploaded to the athlete’s USA Boxing membership account.

The Masters Physical Examination Form is required for athletes age 40 and older who intend to spar or compete. Once completed, the form must be uploaded to the athlete’s USA Boxing membership account.

Athletes who have undergone LASIK surgery and plan to compete must complete this release form and upload it to their USA Boxing membership account.

Athletes with dental braces who plan to compete must complete this form and upload it to their USA Boxing membership account.

Athletes with breast implants who plan to compete must review, complete, and submit this consent waiver. Once completed, the document must be uploaded to the athlete’s USA Boxing membership account.

This form serves as a pregnancy declaration and liability waiver confirming that the athlete is not pregnant at the time of competition. Athletes or their parent/legal guardian must complete and upload this form to the athlete’s USA Boxing membership account prior to participation.
